Mastopexy is a breast lift surgery performed in women after series of weight loss, pregnancy, menopause or lactation. The simple method of mastopexy involves removal of skin from around the areola. This may also be used to lower the size of the areola if it is already too large. The surrounding skin is then gathered and sutured to the areolar tissue. The scar is limited to around the areola, and since it is at the junction of the dark and light skin, it tends to be well camouflaged. This surgery fell out of favor because invariably there would be secondary stretching of the areola within a year or so of the procedure. The method has been modified to incorporate a purse string suture that supports the bunching up of tissue and has been very helpful in over coming many of the problems of the original surgery. This operation is currently one of the most common types of mastopexy which is performed. During the 1st year after operation, there is a visible puckering and bunching around the incision. This is quite variable, but while it smoothes out, it is quite noticeable at first. More important, in the long term, the mastopexy suffers from the fact that there remains a somewhat unnatural fullness beneath the breast and in the lateral quadrant that is the result of not removing skin from under the breast. Combining the lift with the use of a small implant minimizes some of these disadvantages. The surgery is usually carried out only in conjunction with augmentation. This is one of the most popular mastopexies, because of its relative simplicity and the avoidance of scars beneath the breast. For more advanced types of breast sagging (ptosis), skin is removed from around the areola and from the breast’s bottom, allowing an upward repositioning of the nipple complex as well as a tightening of the lower breast. Although this leaves some scarring beneath the breast, the improvement in shape and firmness usually represents a satisfactory trade off. There are various kinds of surgeries exist, but a scar around the areola as well as a vertical scar running from the lower edge of the areola downward to the base of the breast characterize all of them. There is also another scar in the crease beneath the breast in many cases. Possible complication of Mastopexy involves scarring, infection, asymmetry of the breasts, loss of sensitivity of the nipple, skin necrosis.
